Why this form matters. The Compensation Fund classifies every employer into an industry sub-class that determines the annual assessment rate. The sub-class must reflect the actual nature of your business activities. MAPD Accounting will register your business with the Compensation Fund using exactly the sub-class(es) you declare below. Please read each sub-class description carefully before selecting. A reference list of all classes and sub-classes is available at the bottom of this form.

  1. The information provided in this form is true, complete and accurate as at the date of declaration.
  2. I have read and understood the description(s) of the COIDA sub-class(es) selected in Section 4 and confirm that they reflect the actual nature of the business activities.
  3. I am duly authorised to make this declaration on behalf of the business named above.
  4. I instruct and authorise MAPD Accounting to register the business with the Compensation Fund using the sub-class(es) declared in Section 4 of this form.
  5. I understand that the sub-class determines the annual COIDA assessment tariff payable by the business, and that the Compensation Fund may independently re-classify the business if its inspection finds the activities differ from those declared.
  6. I acknowledge that any subsequent request to amend, correct or change the sub-class registered (whether due to error in this declaration, change in business activities, or any other reason) will be treated as a separate billable amendment by MAPD Accounting, charged at MAPD's prevailing rates.
  7. I indemnify MAPD Accounting against any penalties, interest, back-assessments or losses arising from the sub-class declared herein being subsequently found to be incorrect or incomplete.
  8. I consent to the processing of the personal information provided in this form in accordance with the Protection of Personal Information Act, 2013 (POPIA), for the purpose of effecting the COIDA registration and related services.
