Complete this questionnaire to start the registration of a new inter vivos trust. The information you provide allows MAPD Accounting to draft your trust deed and all supporting documents required by the Master of the High Court.

How this works. Tell us about the trust you want to set up — the founder, trustees, beneficiaries and how it should operate. MAPD Accounting will use exactly these details to draft the trust deed and the Master’s registration documents (J401, J417 trustee acceptances, J450 beneficiaries declaration and supporting affidavits). A member of our team will then arrange signing and lodgement. Fields marked * are required.

List every trustee. A family trust usually requires at least one independent trustee — someone who is not a beneficiary and not related to the family. Add as many as you need.

List every beneficiary. If a beneficiary is a minor or mentally incapacitated, guardian details are required.
